  3. In Column B, input the total payroll specifically for employees working in Whitley County during the same period.
  4. Calculate the payment due each quarter based on the amount listed in Column B (1% of Column B) and enter this in Column C.
  5. Total Columns A, B, and C. For Line 1, enter the total tax withheld as per W-2s. If any tax was paid but not withheld, add that amount on Line 2.
  6. Add Lines 1 and 2 together for Line 3. Finally, subtract Line 3 from the total of Column C to determine any over or underpayment on Line 4.
  7. Ensure you include an employee listing or copies of W-2s as required before submitting your form.
