2. Begin by filling out the General Information Section. Provide your first name, age, gender, ethnicity/race, job title at the social enterprise, and the name of the social enterprise or AJCC/WSC.
3. Choose between two options: Option 1 allows you to submit a written testimonial in 1-2 paragraphs using guiding questions, while Option 2 requires you to complete the questionnaire without writing a testimonial.
4. If you choose Option 2, answer each question thoughtfully. Reflect on your challenges before joining LA:RISE and share insights about your experience and future goals.
5. Review all responses with the participant to ensure accuracy and completeness before submission.
6. Email the completed form along with any signed consent forms and photos to larise@redf.org.
