4. Provide the Agency Item No, Record Series Title, and a detailed description of items. This should include information about what the records pertain to and any relevant document names.
5. Indicate the Latest Fiscal Year of Records. For each box being transferred, complete a separate form as required.
6. For RECORDS RETENTION DEPARTMENT ONLY, fill in Date of Transfer, Work Order No (if applicable), File Box No, Box Location, Longest Retention Period, and Fiscal Year for Destruction.
7. Finally, check if Archival Review is required and complete Document Type 2 if necessary.
