3. Move to Section 2 for your prospective employer to fill out their details including name, address, job title, and a description of duties. They should also indicate the start and end dates of employment along with their signature.
4. In Section 3, have your academic advisor describe how this training supports your academic goals. They will need to sign and date this section as well.
5. Finally, in Section 4, the executive officer must confirm the nature of your proposed activity and eligibility for Curricular Practical Training by selecting appropriate options and signing off on the form.
