  1. Click ‘Get Form’ to open it in the editor.
  2. Begin by filling out the 'Information About You' section. Enter your full name, birth date, and either your Social Security Number (SSN) or ETF ID. If you are a beneficiary, include the original participant’s SSN or ETF ID.
  3. Next, move to the 'Address' section. Input your current or new mailing address, including city, state, and ZIP code. If this is an address change, check the appropriate box and provide your previous address details.
  4. If applicable, complete the 'Name Change' section by entering your previous name and your new name.
  5. In the 'Contact Information' area, provide your primary and alternate phone numbers along with your email address.
  6. Finally, sign in the 'Authorization' section to authorize ETF to update your account with the provided information. Don’t forget to include today’s date.
  7. Once completed, mail or fax the form to ETF as instructed at the bottom of the document.
