  6. Section 6 requires you to choose your preferred payment method: direct credit into your bank account or a warrant (cheque). Fill in the relevant details accordingly.
  7. Complete section 5 with your name and address for correspondence. Ensure this is accurate for timely communication.
  8. Finally, sign and date the form in section 7. If under 16, a parent or guardian must sign and indicate their relationship.
