A condition of imminent brain death requires either a Glasgow Coma Score of 3 and the progressive absence of at least three out of six brain stem reflexes or a FOUR score of E0M0B0R0.
The highest possible GCS score is 15, and the lowest is 3. A score of 15 means youre fully awake, responsive and have no problems with thinking ability or memory. Generally, having a score of 8 or fewer means youre in a coma. The lower the score, the deeper the coma is.
A Glasgow Coma Scale (GCS) score of 3 on presentation in patients with severe traumatic brain injury due to blunt trauma has been recognized as a bad prognostic factor. The reported mortality rate in these patients is very high, even approaching 100% in the presence of fixed and dilated pupils in some series.
The GCS is scored between 3 and 15, 3 being the worst and 15 the best. It is composed of three parameters: best eye response (E), best verbal response (V), and best motor response (M). The components of the GCS should be recorded individually; for example, E2V3M4 results in a GCS score of 9.
The use of three as the lowest possible score could be termed a suspended zero as a patient who scores three is deeply comatose but not dead. Thus it is appropriate that the lowest possible score in the GCS is three rather than zero as the patient is deeply comatose rather than dead.
