The Georgia Immunization Registry (GRITS) is designed to collect and maintain accurate, complete and current vaccination records to promote effective and cost-efficient disease prevention and control.
You can request a copy of your immunization records online at no cost through the Georgia Department of Public Health. Your healthcare provider or local public health department may also be able to provide you with a copy of your immunization records.
Georgia Immunization Registry law , passed in 1996 and expanded by House Bill 1526, requires reporting by any person who administers a vaccine or vaccines licensed for use by the United States Food and Drug Administration to a person. Several options are available for submitting immunization records to the Registry.
To obtain the school immunization forms required, take your childs personal immunization record to any local county health department or Georgia physician and they can complete the required forms and give any required vaccines.

Immunization Certificate: You must submit proof of required vaccines with an Immunization Certificate (Form 3231). Your local health department or physician can complete the form for you.
TO gain access to the information contained in the GRITS system you must contact the GRITS Enrollment Coordinator at 404-463-0810 or e-mail immreg@dhr.state.ga.us.
This form may be obtained from the Forsyth County Health Department located at 428 Canton Hwy, Cumming, GA 30040 (770-781-6900) or a Georgia physician. Students coming from another state or country should contact a Georgia Health Department or Georgia physician to have their immunizations transferred to GA Form 3231.
