1. Click ‘Get Form’ to open the EPC form in the editor.
2. Begin with Part A, where you will enter the referring GP's details. Fill in the Provider Number, Name, Address, and Postcode.
3. Next, input the Patient details including their Medicare Number, Reference Number, First Name, Surname, Address, and Postcode.
4. Specify the allied health professional by entering their name or type in the designated field. Ensure to include their Address and Postcode.
5. Indicate the number of services required by writing in the ‘No of services’ column next to each relevant Allied Health Professional (AHP) type listed.
6. In Part B, after each service is provided, have the servicing allied health professional complete their section by entering their HIC Provider/Registration Number and signing.
7. Finally, follow Part C instructions for claiming benefits. Ensure all originals and photocopies are signed appropriately before submission.
