How to use or fill out the EPC form with our platform

- Indicate the number of services required by writing in the ‘No of services’ column next to each relevant Allied Health Professional (AHP) type listed.
- In Part B, after each service is provided, have the servicing allied health professional complete their section by entering their HIC Provider/Registration Number and signing.
- Finally, follow Part C instructions for claiming benefits. Ensure all originals and photocopies are signed appropriately before submission.
