  3. Provide the name and street address of your resident agent. Remember, a Rhode Island street address is required, not a P.O. Box.
  4. Select how your company will be treated for federal income tax purposes by checking the appropriate box: partnership, corporation, or disregarded entity.
  5. Fill in the principal address of your LLC. If unknown, type 'not yet determined.'
  6. Optionally include any additional provisions agreed upon by members in the designated section.
  7. Indicate how the LLC will be managed by checking either 'by its members' or 'by one or more managers,' and provide their details if applicable.
  8. Check the box for 'Date received' unless you want a later effective date within 90 days.
  9. Ensure an Authorized Person signs and dates the form before submission.
