Requirements that apply to all controlled substance records required to be kept: Must be complete and accurate. Must be stored at the registered location. Must be kept for two years.
Each inventory shall contain a complete and accurate record of all controlled substances on hand on the date the inventory is taken, and shall be maintained in written, typewritten, or printed form at the registered location. An inventory taken by use of an oral recording device must be promptly transcribed.
Date, time, drug, quantity, and method of disposal must be logged and one or more persons must initial or sign the log as witnesses.
Controlled Substance Recordkeeping. All controlled substance records are required to be retained for two years following full use, transfer, or disposal. These records include purchasing and shipping records, inventories, usage and disposal logs and disposal paperwork.
