TIME CARDS When you fill out your time card, you must indicate the time you started working and the time you stopped. When you fill out a time card, you do need to indicate what activities you performed.
Time cards should be completely filled in and signed by both the employee and consumer the last day worked in the pay period. Do not sign before last day worked. If you do not know your UMPI number or your consumer Subscriber IDplease call the office and we will provide you with this information.
PCA agencies must have documentation supporting that the service has been provided for both individual PCA and QP services.
