  4. Select the Financial Statement Type from the options provided: Compilation, Audit, or Review. This selection must correspond with the financial statement submitted.
  5. Fill in the financial figures: Total Current Assets, Total Assets, Total Current Liabilities, and Total Liabilities. These should reflect your institution's financial status as per Generally Accepted Accounting Principles.
  6. Enter the Tuition Revenue Generated in Florida. If applicable for non-degree institutions, list Tuition Revenue for each licensed location separately.
  7. Finally, provide the Print Name of the CPA who prepared the financial statement and ensure they sign where indicated. The Institutional Representative should also print their name, sign, and date the form before submission.
