Next, provide details about the supervising physical therapist, including their name, license number, state of licensure, email, and phone number.
Indicate whether you were paid or volunteered during your experience and select the type of experience (inpatient or outpatient).
In the section for physical therapy specialty areas observed, enter the hours spent in each area. Be thorough to ensure all experiences are documented.
Finally, input the total number of hours over the span of your experience along with start and end dates before obtaining a signature from your supervising PT.
