Review all entries for accuracy and clarity. Initial any corrections made on the form to prevent disputes later.
Once completed, print the form and sign it in ink using a color other than black. Ensure that no attachments are included.
Mail or fax the completed form to TRS at the provided address or number; do not submit it through your employer.
