Form W-4 tells you the employees filing status, multiple jobs adjustments, amount of credits, amount of other income, amount of deductions, and any additional amount to withhold from each paycheck to use to compute the amount of federal income tax to deduct and withhold from the employees pay.
Form W-4 comes in a four-page packet. However, the form itself is just one page and has only five steps.

Multiple jobs or spouse works (Step 2) This section is for if you work multiple jobs at the same time or are Married Filing Jointly and both you and your spouse are employed. To be accurate, both spouses should fill out the new Form W-4 for each job. The form lists three ways you can complete this section.
If you are looking for 1099s from earlier years, you can contact the IRS and order a wage and income transcript. The transcript should include all of the income that you had as long as it was reported to the IRS. All you need to do is fill out a Form 4506-T and mail or fax it off to the IRS.
