5. Indicate any major component parts that were missing prior to acquisition by circling the appropriate letters (F=Frame, E=Engine, etc.) for each vehicle listed.
6. Attach proof of ownership and lien releases for each vehicle to ensure compliance with regulations.
7. Review all entries for accuracy before signing the certification statement at the bottom of the form.
8. Mail the completed form by the last day of each month to Title Bureau, PO Box 2105, ESP, Albany, NY 12220-0105. Keep a copy for your records.
