2. Begin by filling out Block 1 with the name of the party, corporation, or organization that will receive the certificate. Ensure accuracy as this name will appear on the certificate.
3. In Block 2, check the appropriate box for the type of application you are submitting: Type Certificate, Supplemental Type Certificate, or Production Certificate.
4. Complete Block 4 with the address of the applicant. Note that a post office box is not acceptable.
