In Section 1, enter your employer information including company name, owner name, mailing address, telephone number, fax number, email address, federal employer identification number (FEIN), and DEW account number. If you lack a DEW account number, complete and submit Form UCE-151.
Proceed to Section 2 and fill in the representative's details such as their company name, authorized representative name, mailing address, telephone number, fax number, and email address.
In Section 3, check the appropriate boxes to indicate what information you authorize your representative to receive. If all UI tax matters apply, simply check the first box.
For Section 4, specify any previous authorizations that should remain effective and attach copies of those documents if necessary.
Finally, in Section 5, sign and date the form. Ensure you have the authority to execute this Written Authorization on behalf of your business.
