The filing fee for a Name Change case is $314.00. Minors cannot be included in the same case as an adult, a separate case for Minor Name Change must be filed. Call the Circuit Clerk Civil Division at (815)334-4310 to request a court date.
When asking for a divorce, you can include a name change request in the petition and order. After the divorce, you can then take a certified copy of the divorce order (also called a Judgment for Dissolution of Marriage) to government agencies to update your name on legal documents and ID.
After court, you will be instructed to return to the Circuit Clerks office to obtain certified copies of your Name Change Decree. The fee is $6.00 for the first copy and $15.00 for each additional copy.
After your divorce You will need the case number and name, and county where it was filed, to fill out your request. Fill out Ex Parte Application for Restoration of Former Name After Entry of Judgment and Order (form FL-395) (opens in a new tab). This asks the court to change your name back to a former name.
