  4. For sections 19 and 20, input the ADDITIONAL DEBTOR’S EXACT FULL LEGAL NAME. Choose either an organization’s name or an individual’s surname and ensure you do not abbreviate or combine names. Include their mailing address details accurately.
  5. Repeat step 4 for section 21 if there is a third additional debtor. Ensure all information is complete and correct.
  6. In sections 22 and 23, enter the ADDITIONAL SECURED PARTY’S NAME following the same format as previous steps. Make sure to include their mailing address as well.
