In Section 1, provide details about the Other Party. Select whether they are a Respondent or Petitioner, then fill in their full name, date of birth, and driver's license number.
Complete the demographic information including race and sex. Specify their relationship to any children involved in this case.
Fill in the mailing address, noting that this will not be kept private. If applicable, indicate if their home address is the same as their mailing address.
Continue by providing contact details such as phone number and email address. If required, include their Social Security number and employer's information.
Repeat steps 3 to 6 for any additional parties listed in Section 2.
In Section 3, enter details for any children involved. You may skip Social Security numbers if your case only concerns a protection order.
