STROKE THROMBOLYTIC CHECKLIST. This checklist is intended as a tool for the pre-hospital identification of patients who may benefit from the administration of thrombolytics for acute stroke.
Ischemic Stroke: The only blood test that is necessary before tPA usage is the blood glucose level.
What are thrombolytic drugs? Thrombolytics (fibrinolytic drugs) are clot-busting drugs that break up and dissolve blood clots that get in the way of your blood flow. Thrombolytics are enzymes that kick off the process of breaking down proteins (fibrins) that form clots.
FDA Approved Indications Ischemic Stroke: The only blood test that is necessary before tPA usage is the blood glucose level. If the patient is on anticoagulation like coumadin, then only we should do PT, PTT, and INR, etc. The benefit of tPA depends a lot on time.
1. Intravenous r-TPA (0.9 mg/kg, maximum 90 mg) with 10% of the dose given as a bolus followed by an infusion lasting 60 minutes is recommended treatment within 3 hours of onset of ischemic stroke (Grade A recommendation).
