8. Complete Block 6 by detailing Acres to be Treated, Number of Applications, Number of Sites, and Specific Descriptions of Sites.
9. Fill out Block 7 with Month(s) of Year and State(s) where treatment will occur.
10. In SENSITIVE AREAS (Block 8), identify areas to be avoided and those requiring caution during treatment.
11. Finally, provide any additional REMARKS in Block 9 that may assist in evaluating your project before submitting for approval.
