Creating a Check Box Open the PDF file that you will use for the form. From the Forms menu, select Add or Edit Fields From the Add New Field pull-down menu, select Show Tools on Toolbar. Click Checkbox Tool. Position the cursor where you want the form field to begin.
First of all, go to menu Edit (Win) or Acrobat (Mac) - Preferences, choose Forms category and make sure that Automatically detect Form fields option is enabled. After that, go to menu File - Create - Create Form and then select your Word file and click on Start.
Check the individual fields by double-clicking on one of the fields and in the Properties dialog under the General tab there is a check box for Read Only. Is it checked? If it is uncheck it and uncheck any others set to Read Only.
Form Check Box Start the Form Editing Mode by going to the Forms Tab. Click on the Check Box button located on the toolbar. Using the mouse move to the location you want to place the field and then click to place the field or click and drag to create a custom field size.
If editing an existing checkbox, then place a cursor over it and click a right mouse button. Select Properties from the popup menu. Select the Options tab on the Check Box Properties dialog. Select a check box style.

Go to Tools Form Check Box or press (Alt+Q) to open the Forms panel, and then select Check Box from the top row. Click-and-drag where youd like to place the check box on your form.
