Click the Insert tab. Click the Header button on the ribbon. Choose the first option, Blank. Word inserts a blank header showing [Type text] at the top of the document. Double-click the [Type text] wording and type your initials.
Write out their first name and surname, then add an M.D. suffix at the end, which notes the formal nature of whatever youre filling out.

If you are asked to respond to a medical doctor, address the letter and envelope to Edward Smith M.D., but the salutation should read, Dear Dr. Smith.
A: In the United States, the use of academic titles, such as M.D., is generally permissible as long as its clear and truthful about ones qualifications and does not mislead the public. Since you earned your M.D. degree, you can include it after your name on the book cover.
