Under 'Forms for Supervisors,' locate the Supervisor Performance Evaluation Form. You should have received your employee's self-evaluation via email, which you'll need for the final evaluation. Start by entering the employee's information: first and last name, email address, colleague ID (found on their self-evaluation), department, your email, and the review period. In Section One, goal setting, input two to four goals aligned with departmental and employee objectives. Remember, if goals change throughout the year, update them to maintain alignment. Goals should be SMART: specific, measurable, attainable, relevant, and timely. Section Two focuses on writing ratings for the evaluation.
