If you are filing taxes for an individual mandate state and do not have a copy of your 1095B, you may download one immediately from your member website or request one by calling the number on your ID card or other member materials.
How to find your 1095-A online Log into your Marketplace account. Under Your applications, select your 2023 application not your 2024 application. Select Tax Forms. Download all 1095-As.
Call the number on the back of your BCBSIL member ID card to ask for your Form 1095-B to be mailed to your address on file within 30 days. If you cant find this number, you can also call 1-855-710-6984. Once this form is received, your Form 1095-B will be mailed to your address on file within 30 days.
