This Management Information System (MIS) form is made-up of four sections: employer information; covered employees (i.e., employees performing DOT regulated safety-sensitive duties) information; drug testing data; and alcohol testing data. The employer information needs only to be provided once per submission.
In most cases, records containing the information required under the final rule must be maintained for 2 years from the date you created or obtained the records. One exception is for records maintained by an RFE or restaurant for food purchased directly from a farm; those records must be maintained for 180 days.
FAA Requirements 2 Years: Education and training records. Records related to the alcohol and drug collection process. 3 Years Previous employer records. 5 Years: Annual MIS reports.
For every vehicle in their control for 30 days or more, DOT-regulated companies must retain the following records for one year at the location where the vehicle is garaged and for six months after the vehicle leaves the companys control: Identifying information (company number, make, serial number, and tire size)
The length of time a failed drug screen stays on a record depends on the employer and the type of record. Records from a failed DOT drug test remain in the Drug and Alcohol Clearinghouse Database for five years or until a person resolves the violation through the return-to-duty process and follow-up testing.
