Begin with BLOCK 1: Home Information. Fill in the Manufacturer Name, Address, Model, License Number, Total Square Feet, Date of Manufacture, Label/Seal Number, Complete Serial Number, Weight, and Size. Ensure all fields are completed clearly.
Proceed to BLOCK 2: Lienholder and Borrower Information. Enter the Name and Address of the Lienholder followed by the Consumer's Name, Address, City, State, Zip Code, and Phone number.
In BLOCK 3: For Release of Lien, specify the Release of Lien Effective Date and Date of Repossession.
BLOCK 4(a) requires you to check one method for Foreclosure of Lien Information. If applicable, attach necessary documents as indicated.
If foreclosure is being recorded, complete BLOCK 4(b) by checking one method of Sale and providing retailer details if required.
Finally, in BLOCK 5: Notarized Signature Required, ensure that the authorized person signs and dates the document before a notary public.
