To order a crash report by mail, fill out the Crash Report Request (TR-67A) form and mail it along with payment (see fee below) to the address listed on the form. The $3 fee is not required for mail-in requests. Please allow 10 business days from the time of receipt for the request to be processed.
The only time you do not need to call your insurance company is when the accident happens on your own property, involves no other drivers (so no one else may be at fault) and the damage to your vehicle is minimal or at least of an amount you can afford to cover on your own or lose altogether.
Steps To Take After a Car Crash Stay at the scene, but move to safety if possible. Seek medical care, if necessary. Take photos and videos of the vehicle(s) and accident scene. Call the police to file a report. Obtain insurance and contact information from other motorists involved, as well as bystanders.
Complete and mail the MV-104 form to the address listed on page two. If you do not file a crash report, DMV may suspend your driving privileges until the report is on file.
You or your insurance agent, broker, or legal representative must complete an SR-1 report and send it to DMV within 10 days if someone is injured (no matter how minor the injury) or killed, or property damage is over $1000.
