2. Begin by entering your BOMI International ID#, application date, name, email, and preferred mailing address in the designated fields.
3. List your employment history starting with your current employer. Ensure you include all relevant details such as company name, verifier's name and title, address, phone number, position(s), and dates employed.
4. For each employer providing verification, duplicate the Criteria Verification Checklist form. Have your verifier check all applicable criteria that you have met over the three-year period.
5. Ensure that U.S. verifiers notarize their signatures on the checklist while Canadian verifiers should provide their email addresses for authentication.
6. Compile all completed forms along with your resume and submit them to BOMI International for processing. Remember that incomplete applications will not be accepted.
