  4. For each month from January to December, enter the total tax withheld as reported. Make sure to add these amounts together and enter the total on Line 13.
  5. Next, input the tax withheld per W-2 statements on Line 14a and per 1099 statements on Line 14b. Add these two lines together and enter the result on Line 15.
  6. Compare Lines 16 and 17 to determine if there is an overpayment or additional tax due. Fill in Lines 18 and 19 accordingly.
  7. Complete any remaining fields such as interest and total amount due before signing and dating the form.

After submitting the form electronically: Each taxpayer and representative will receive an email from requesting an electronic signature. Once all required signatures are received then the form will be sent to NCDOR for processing.
