Click ‘Get Form’ to open the btec sa16 form in the editor.
Begin by entering the Centre Number and Centre Name at the top of the form. This identifies your institution.
In the 'Tick (✓) action needed' section, select whether you want to add a unit, delete a unit, or amend a grade by checking the appropriate box.
For adding a unit, enter the new Unit Code and Award Grade in the corresponding fields after ticking 'Add unit'.
To delete a previously reported unit, tick 'Delete unit' and provide the Unit Code and Award Grade for that entry.
If amending a grade, check 'Amend grade' and fill in the Unit Code along with the revised Award Grade.
For making an award application, ensure all requirements are met before entering 'Y' in the ‘Award Appl’ box and providing the learner completion date.
Complete your declaration by filling in your name, signature, phone number, date, and position on behalf of your centre.
