Form W-2, also known as the Wage and Tax Statement, is a document an employer sends to each employee and the Internal Revenue Service (IRS) and shows income earned from an employer and the amount of taxes withheld from an employees paycheck so taxpayers can file federal and state taxes.
Health Insurance Cost on W-2 - Code DD. Employers are required to report the cost of an employees health care benefits in Box 12 of Form W-2 Wage and Tax Statement, using code DD to identify the amount. This amount is reported for informational purposes only and is NOT taxable.
Box D reports the control number, which uniquely identifies your W-2 Form from your employers records. This number is usually assigned by your employers payroll processing software. You might need this number if youre trying to import your W-2 form to an eFiling platform like Tax1099.
