  6. Proceed to 'Other Capabilities' and check boxes that apply regarding lifting weights and other physical tasks.
  7. Complete sections on upper and lower extremities by answering questions about dominant hand usage and repetitive actions.
  8. Fill out any work environment restrictions in section 4 and additional restrictions in section 5 as applicable.
  9. Finally, provide comments regarding any known problems that may affect the employee's return to work in section 6.

The Work Capability Assessment is used to decide whether or not you are fit for work for ESA and Universal Credit.
Form OWCP-5c, Work Capacity Evaluation, is used by healthcare providers to assess an injured workers ability to perform their job after a musculoskeletal injury. This form is crucial for determining any work restrictions and ensuring appropriate accommodations are made for the injured workers return to work.
If you get Universal Credit and you cant work because youre disabled or have a health condition, youll usually get an extra form to fill in. The form is called the work capability questionnaire or UC50. The Department for Work and Pensions (DWP) use the form to decide whether you: have to work or look for work.
After applying for a Work Capability Assessment, any decision can take up to 12 weeks. But its often longer. In some cases, it can take many months. Application decisions also depend on which funding pot the support comes from.
This form, when completed, is used to enable an employer to accommodate an ill or injured employee to remain at, or if absence is unavoidable, to return to work as soon as they are safely able to do so.
