4. If available, in Part C, provide details about the spouse of the deceased retiree. Again, adhere to the required formats for phone numbers and Social Security Numbers.

6. After completing all sections, click ‘Print’ to save a copy for your records before clicking ‘Submit’ to send the form to DFAS.
