Next, complete Applicant B’s details similarly. If only one parent is applying, provide a reason in Section 3.
In Section 4, enter the child's details as they appear on their birth certificate. This includes first names, surname, date of birth, and place of birth.
Proceed to Section 5 to propose the new name for the child. Clearly print both first names and surname without using block letters.
Fill out Sections 6 through 10 regarding parents' names at the time of birth/adoption and reasons for the name change. Be detailed to avoid delays.
Complete the declaration section by signing and having your signature witnessed as required in Section 12.
