In Section 2, input the Date of resolution. Ensure you format it correctly as dd/mm/yyyy.
For consolidation, sub-division, or redemption, provide details for each class of shares. Specify the previous and new share structure including nominal values and number of issued shares.
Complete the Statement of Capital sections (7 and 8) to reflect your company's issued capital following changes. Include amounts paid up on each share and any unpaid amounts.
Review all entries for accuracy, ensuring that all mandatory fields are filled out before proceeding to sign the form.
Finally, add your signature in Section 11 and provide any optional presenter information if desired.
