By signing Form 2, the spouse/common-law partner acknowledges that he or she is giving up his or her entitlement, under the Pension Benefits Standards Act Regulations, to receive the balance of what would have remained in the LRSP, LIF, RLSP or RLIF account at the time of the planholders death.
The Form 2 order is directed to the police in the same locality where the justice has jurisdiction and provides authority to the police to take the person named in the order into custody forthwith to an appropriate place where the person may be detained for examination by a physician.
Note: Two people who live together in a conjugal relationship (or common law relationship) outside marriage can sign a Joint Declaration of Conjugal Relationship (Form 3, Change of Name Act) to change their last name to their spouse/ partners last name (Form 1, Change of Name Act).
