5. Complete your personal information in the Authorize section. Include your Full Name, Address, Phone Number, and Email. If using a different shipping address for replacements, provide that as well.
6. Finally, sign and date the bottom of the form to authorize refunds and charges. Ensure all fields are filled clearly before submitting your return shipment.
