3. Fill in your Cardholder Name and Mailing Address, including Street, City, State, and Zip Code.
4. Indicate whether you requested the card and select the type of card (Credit Card, Debit Card, or ATM Card).
5. Provide your Home Phone and Work Phone numbers for contact purposes.
6. Specify if your card was in your possession at the time of fraudulent transactions or if it was lost, stolen, or never received.
7. Answer whether law enforcement was notified about the fraudulent activity.
8. Enter important dates: when you discovered the fraud, reported it to your credit union/processor, and when the first fraudulent transaction occurred.
9. Detail any unauthorized transactions on a separate sheet if necessary and provide total amounts.
10. Sign and date the form at the bottom to confirm accuracy and consent for information release.
