Select your relationship to the patient from options provided (e.g., Parent of Minor Child or Patient Representative of Adult Patient).
Check one option under 'The purpose of this request' to indicate whether it's for new access or renewal.
Review all entered information for accuracy before signing and dating at the bottom of the form.
