  4. Enter your Importer Name in Block 3. For individuals, list last name first followed by first name and middle initial. For businesses, input the business name first.
  5. Complete Blocks 4 and 5 if applicable, indicating any division names or alternate business names.
  6. Fill out Block 6 by checking the type of company you represent: Corporation, Partnership, Sole Proprietorship, etc.
  7. Provide your mailing address in Block 7 and ensure to include city, state code, ZIP code (Blocks 8-10), and country ISO code if applicable (Block 11).
  8. If your physical location differs from your mailing address, complete Block 12 with that information.
  9. Finally, certify your information by signing in Block 20 and entering the date in Block 21 before submitting.
