4. Review the authorisation statement carefully before signing. Ensure that the signature matches what is on record with your bank. If required, include your company’s stamp.
5. For Part II, obtain endorsement from your bank. This section must be completed by an authorised bank officer who will certify that the details match their records.
6. Once both parts are completed and signed, mail the original form to AGD AP Branch at the specified address. Remember, fax copies are not acceptable.
