  6. If applicable, fill out Part E for Legacy Child Certification. The Veteran must waive rights to unused hours and provide their signature along with their child's signature.
  7. Review all sections for accuracy before submitting the completed form along with required documentation to your college or university's Hazlewood Act Exemption administrator.
