3. In Part B: Description of the Waste, describe the process that generated the waste (Field 1) and provide the Standard Industrial Classification (SIC) code (Field 2). For each type of waste collected, complete all required fields including EWC code, quantity, physical form, hazard codes, and container details.
4. Proceed to Part C: Carrier’s Certificate. Enter carrier details including name and registration number. Ensure you certify that all information is correct and that special handling requirements are noted.
5. Complete Part D: Consignor’s Certificate by providing your name and signature along with date and time. Finally, fill out Part E: Consignee’s Certificate for confirmation upon receipt of waste.
