1. Click ‘Get Form’ to open the CAP Observation Form in the editor.
2. Begin by selecting the appropriate checkboxes for the Observation Visit, indicating whether it is announced or unannounced for each relevant section (1.A.4, 1.B.2, and 2.A.3).
3. Enter the Date of Lesson and Time (start/end) in the designated fields to ensure accurate documentation.
4. Fill in the Content Topic/Lesson Objective by selecting from options such as Whole Group, Small Group, One-on-One, or Other.
5. Document any Active Evidence Collection that occurred during the observation by synthesizing and categorizing evidence under each element listed (e.g., Well Structured Lessons, Safe Learning Environment).
6. Provide focused feedback by detailing strengths in the Reinforcement Area/Action and areas for improvement in the Refinement Area/Action.
7. Once completed, ensure that the original and filled-out Observation Form is returned to the candidate for inclusion in their Licensure Application Packet.
