4. Proceed to SECTION C: SERVICE USER DETAILS ONLY. Select the relevant service type by ticking one option and provide details about specialties involved.
5. For SECTION D: STAFF MEMBER/AGENCY/PANEL, indicate the category of person involved and complete any necessary fields.
6. Complete any additional sections as required based on your role (e.g., external contractor details) and ensure all fields are filled accurately before submission.
