Academic units wishing to request grade changes must submit a Grade Change and Academic Record Update Form to the Registrars Office for processing. The form must contain the signature of the respective academic Dean in order to be processed.

Complete Description: State the grievance with explanatory or descriptive material. List all informal efforts undertaken to resolve the grievance and the results.
After a student has proceeded through the normal procedures for requesting school credit toward their degree, the student may appeal the decision within 10 days of the decision by completing and submitting an Ombuds Student Data Form to the Office of the University Ombuds.
The most common examples of grievances filed at a school level include but arent limited to: Insufficient supplies available to students. Unsafe conditions. Insufficient and/or missed prep. Failure to adhere to a students IEP. Violations of Title IX.
